The report he just sent to the Justice Department names the biggest children's hospitals in America – and accuses every one of them of the same scheme.

Vance Sends DOJ A List Of Nearly 100 Hospitals

Vice President JD Vance dropped a letter on Attorney General Todd Blanche's desk Thursday that should terrify hospital executives from Boston to Los Angeles.

The letter came attached to a 64-page HHS report with a title that doesn't leave much to the imagination: "Wolves in White Coats: How Doctors and Hospitals Pushed and Profited from the Fraud of 'Gender Medicine.'"

Vance didn't mince words. "Some pediatric gender clinics and children's hospitals have apparently used misleading or even fraudulent diagnostic and billing codes to ensure that insurance companies will cover the procedures."

Translate that out of bureaucrat-speak. Hospitals allegedly lied on insurance paperwork so they could keep cutting and drugging children – and get paid for it.

The list of hospitals referred for investigation reads like a who's who of prestigious American medicine – Boston Children's Hospital, Children's Hospital Los Angeles, Children's Hospital of Philadelphia, NYU Langone, Vanderbilt, Seattle Children's, Rady Children's.

Mount Sinai Medical Center billed $8.2 million. Boston Children's billed $6.5 million. These aren't fringe clinics. These are the institutions parents trust with their kids' lives.

Behind that $8.2 million is a real pattern: confused teenagers who needed a therapist and got a hormone shot instead.

The Fraud Wasn't An Accident. It Was A Business Model.

Here's how the scheme allegedly worked, and it's simpler than you'd think.

Insurance companies often won't pay for "gender-affirming care." So according to the report, hospitals just didn't call it that on the paperwork.

A doctor prescribes Lupron – a drug originally developed to chemically castrate sex offenders – to a healthy 13-year-old questioning their gender. Instead of coding it as gender treatment, the claim goes in under a vague "unspecified endocrine disorder," or a diagnosis of "precocious puberty" a child doesn't actually have.

HHS pulled the claims data going back a decade. Insurers got billed close to $50 million for puberty blockers using that exact trick, on kids as young as nine.

Zoom out further and the number gets worse. Since 2019 alone, the report puts total billing for these procedures across 225-plus hospitals at nearly $120 million.

That's not a paperwork error repeated by accident hundreds of thousands of times. That's a business model.

Vance put it bluntly: "convincing vulnerable children to reject their gender can create patients for life, earning hospitals tens of thousands of dollars in additional revenue per child."

That's the quiet part hospitals never say out loud – a confused 13-year-old on hormone blockers today is a customer for the next fifty years.

Texas Already Showed Us How This Ends

If you want to know where this goes, look at Texas Children's Hospital.

Whistleblower surgeon Dr. Eithan Haim tried to expose what was happening there back in 2023. The Biden Justice Department didn't investigate the hospital. They indicted Haim, threatening him with ten years in prison for HIPAA violations.

Trump's DOJ dropped the case against Haim four days into the new administration.

Texas Children's wasn't so lucky. The hospital agreed in May to pay $10 million – the second-largest health care fraud settlement in pediatric medicine history – for billing Texas Medicaid using false diagnosis codes tied to gender procedures banned under state law.

Cleveland Clinic settled too, paying $308,000 and agreeing to a 20-year halt on the procedures for minors.

Neither hospital admitted fault. That's how these settlements always work. Write the check, deny everything, move on.

Except now there isn't just one hospital in the crosshairs.

What Comes Next Should Scare Every Hospital Board In America

This is where the Texas case stops being a one-off and starts looking like a template.

The False Claims Act carries triple damages for knowingly false billing to government programs. If DOJ finds what Texas Children's was doing at even a fraction of these other hospitals, we're not talking about $10 million fines anymore. We're talking about a wave of settlements that could run into the billions.

And Blanche's DOJ has already shown it treats this seriously. In January, the department announced more than $5.7 billion in health care fraud recoveries government-wide.

The report's authors made one more point worth sitting with. This scam scaled up under Biden's HHS. His administration used Affordable Care Act rules to threaten hospitals with enforcement if they refused to offer these procedures. Washington didn't just look away. Washington leaned on hospitals to expand it.

CMS already cut off Medicaid funding for pediatric gender-affirming care this week. Now the hospitals that built a decade of revenue on it are staring down federal prosecutors.

The bill is finally coming due.
